We're at 456 E State Street, Suite 1400, American Fork, UT 84003, a few minutes off I-15 between Lehi and Pleasant Grove. Parking is free and the entrance is on the ground floor. Patients drive in regularly from Provo, Orem, Lindon, Draper, and Saratoga Springs.
Yes, in most cases. Utah patients can do their Suboxone or buprenorphine intake by secure video, with follow-ups by telehealth or in person. Some controlled-substance refills require an in-person visit each year, which we'll schedule with you up front.
Our Utah clinic is in the process of transforming to an insurance-based model. We accept several major insurance plans and are awaiting approval for the remaining plans. If we are not yet in network with your plan, we operate as a concierge practice, helping patients submit out-of-network claims for reimbursement. Self-pay rates are kept intentionally low to keep care reachable.
Two things. First, our Medical Director, Dr. Paul Frandsen, has built a Utah practice specifically around medication-assisted recovery, paired with an emphasis on treating the entire body. Second, our American Fork office sits next door to Foundation Wellness, so addiction recovery, hormone health, GLP-assisted weight loss, and integrated mental health can sit in one care plan instead of three.
Foundation Wellness is our sister clinic in the same building. They handle integrated wellness, weight optimization, hormone therapy, and onsite recovery work. There is exciting new research demonstrating the benefits of GLP medications for people dealing with certain types of substance abuse. If you're being treated here for substance use disorder or psychiatric care, your Foundation Wellness team can coordinate directly with us, no records-release lag.
Quantitative EEG brain mapping is part of our larger network and we can refer to or coordinate with the Richmond flagship for qEEG and GeneSight® pharmacogenomic testing if it fits your case. Most Utah-based care happens here in American Fork.
